Effective today, June 19, 2018, I am no longer under contract with Hometown Health as an in-network provider.

You can still use your Hometown Health Plan benefits.

Please note that Hometown PPO plans do not require prior authorization for my out-of-network services, while Hometown HMO plan likely will. We can help you negotiate the process.
