Your Rights & Choices
I protect your health data as the law requires. You have the right to:
- Access/Correct Records: Ask for your records or corrections. Fees apply per ORS 192.563.
- Private Communication: Ask me to reach you in specific ways (e.g., mobile instead of home).
- Limit Sharing: Ask me not to share data for treatment or billing. If you pay out-of-pocket in full, you can stop me from telling your health insurance.
- Accountability: Get a 6-year list of who saw your data. I will tell you if someone steals it.
- File Complaints: Tell me or the HHS Office for Civil Rights if I ignore your rights. I will not punish you.
- Your Choices: Tell me to share data with family, friends, or disaster relief workers who help you.
How I Use Your Data
I use your data to treat you, run my clinic, and bill for visits. The law makes clear rules for when I must or cannot share data:
- Mandated Reporting & Safety: I sometimes must share data without your consent. I do this to report child or elder abuse. I must also do this to stop immediate harm to you or others, or to follow a court order.
- Note on Substance Use Records: Files from drug and alcohol rehab centers get extra safety. I will not share them in court against you unless you agree or a judge demands it.
Extra Privacy Rules
- Reproductive Health: You have a right to legal reproductive care. I will never share your data to help anyone track or punish you for it.
- Signed Promises: I demand a signed form before I share rehab or reproductive records. The person asking must promise to obey the law.

I must keep your data safe, follow these rules, and give you updates if asked.
